Patient Information Data Collection
NHS England is commissioning a modern data service from the Health and Social Care Information Centre (HSCIC) on behalf of the entire health and social care system. HSCIC will collect information from GP practices using The General Practice Extraction Service (GPES). This data will be used to compare, develop and esnure the NHS is providing the best possible services to all of its patients.
The data collected by HSCIC may include family history, vaccinations, diagnoses, referrals, NHS prescriptions and biological values such as blood pressure, BMI and cholesterol. Patient identifiers (date of birth, postcode, NHS number and gender) are required to link GP data with data from other care providers, but this information will be deleted once the link is complete.
Please read the hscic patient information leaflet available in this link or in the practice waiting room, so an informed decision can be made. If you would like further information about this, you can also follow the links below to read the document 'Frequently asked questions for patients', and find out more information about the HSCIC.
If you are happy for your information to be used to improve health services, then you do not need to do anything. However if you do not wish for your information to be used, then please complete the HSCIC data collection disclaimer and return it to the surgery.
